My answer is "duplication".

It's not a technical issue...
It's poor communication between developers and business users.

It's not a technical issue...
No - that leads to requirements being wrong for the business. The code can still be 100% bug free with reference to those requirements. Bugs are a purely technical issue, they are about failure to meet requirements.

Any maybe not just the wrong keystroke by accident, but also by design - when the same words/phrases are used throughout code and get mixed up.
